Embed Referral Form in Affiliate Dashboard

To embed the referral form in the affiliate dashboard: Go to the Look and Feel > Affiliate Dashboard section. 

Here, go to the Dashboard Tabs section > Click on New custom page.

This will open up the Custom page section > set the page title.

Next, you can add instructions to the page.

Now, click on Add section.

Here, select the Lead Capture form option.

After this, select the form. 

You can follow this guide to create the form. 

Finally, click on Save Page.

The form will get embedded in the affiliate dashboard.
